Video Electroencepahogram is used for the diagnosis of epilepsy. Electromyography and Nerve conduction tests are used for diagnosis of nerve and muscle diseases. Transcranial magnetic stimulation is used for management of various headaches and other painful conditions. Transcranial Doppler is used to diagnose obstruction in the flow of blood in intracranial arteries. These investigations are specialised tools handled by experts trained over years.
The Deputy Chief Minister also inaugurated a 1000 LPM (litre per minute) capacity oxygen PSA plant which was donated to the institute by NGO ' Doctors For You'. The expenditure for supply and installation will be borne by the NGO. This plant works on PSA ( Pressure Swing absorption) technique, by which 93% to 94% of pure oxygen is obtained from air. Then it is transported to the patients admitted in hospital through central pipe line system.
Director Prof R K Dhiman welcomed the Hon'ble guest and thanked him for gracing the occasion. Speaking on the occasion, Hon'ble minister appreciated the work culture at SGPGI to treat every patient with utmost care and empathy. He equates medical institutes with temples and doctors as 'GOD'. Ribs are an important source of graft material for bony reconstruction. The rib grafts are used for reconstruction of nose, commonly where the rib provides a stable support for reshaping the nasal shape. Besides this rib grafts are also useful for reconstruction of ear where the rib cartilage provides the framework for the ear.
Taking out the rib is always a challenging and delicate operation where the surgeon has to operated very close to the lungs and there is always the risk of injury during surgery to the lungs. Many techniques are currently practiced for taking out the rib graft but the disadvantage of all these techniques is that after the rib has been taken out from the rib cage there is a large gap that is left behind in the thoracic cage. This large gap makes the rib cage vulnerable to external injury and also may hamper in the respiration of the patient if a large piece of rib is taken or multiple ribs are taken.
A US based company has introduced the Matrix rib fixation system where after the taking of the rib grafts, the defect left can be bridged with the help of very specialized plates and screws. These plates are made of titanium and are specially bent and curved to mimic the shape of the ribs. These plates are very useful to cover the defect left behind after taking out the rib graft.
Such a rib plasty system was used for the first time at SGPGI in a girl with ear deformity. This girl had a lop ear which is a condition in which the upper part of the ear gets folded on the lower part of the ear leading to a unsightly deformity which is quite odd looking and also covers the ear hole.
The rib graft was used to provide support to the corrected ear and was found to be useful in maintaining the corrected position of the ear.
The technique of taking out large parts of the rib without interfering with the function of the remaining ribs opens up new avenues for a large number of patients who have to undergo resection of ribs for making or improving the appearance of many important parts of the face.
The technique of plating the rib requires special training and experience and cannot be performed by inexperienced surgeons. It involves operating on the rib while the patient is under anaesthesia but the chest cavity is continuously moving with the breathing of the patient. It involves drilling holes in rib which is not stationery but is constantly in movement and is therefore tricky and requires utmost care and precision. Moreover due to the presence of the underlying lung there is constant risk of the power drill puncturing the lung during the process of rib drilling. This requires high degree of precision as the lung is quite close to the undersurface of the rib.
Reconstruction of ear was successfully accomplished in this girl from UP who is twelve years old. This opens up lots of possibilities for not only patients who undergo rib reconstruction but is a boon for those patients who have a rib fracture. Traditionally patients with rib fractures are offered no surgical treatment and they have to wait patiently for 4-6 weeks with just chest strapping and wait for the fracture to heal. In this treatment there is a lot of pain even after the chest strapping. The new technique of rib plating overcomes all these problems and can be very useful in patients with chest trauma having single or multiple fractured ribs.
These implants are 1.5 mm in thickness and come with special instruments for plate cutting, bending and contouring. Rib defects of any length and magnitude can be reconstructed with the help of these implants. The plates are specially coloured for right and left side rib reconstruction and have special drill bits to ensure that during drilling the lung is not punctured. The implants are 100% biocompatible and can remain in the body for indefinite period without causing any harmful effects.
The introduction and successful use of rib plasty system at SGPGIMS, Lucknow has opened up opportunities for a large number of patients who require the use of rib grafts for different types of reconstructive surgery for nose, ear or even skull reconstruction and this system is a very important asset and armamentarium in the hands of the plastic surgeon.
The operation lasted four hours and the operating team consisted of
Chief Surgeon: Prof. Rajiv Agarwal, FRCS
Chief Anaesthetist: Dr. Sanjay Kumar
Dr. Divya Srivastava
Resident : Dr. Bhupesh Gogia

Theme: "Tobacco: Threat to our enviornment"
Tobacco use is now a well-documented threat to global health. It kills more than 8 million people a year, including around 1.2 million deaths from exposure to second-hand smoke1, and is currently the world's single biggest cause of preventable death. Tobacco use is linked to at least 20 cancer types and the leading preventable cause of cancer by a wide margin. Indeed, tobacco products cause some 2.4 million deaths globally every year, almost a quarter of all deaths due to cancer (10 million).
Tobacco use is a major issue that impacts all communities and impedes the achievement of Sustainable Development Goals. It has direct harmful effects on its consumer's health, but it also affects the country's economy, environment, women's health and childhood labour. World No Tobacco Day is an annual awareness day taking place on 31st May every year. It was designated by Member States of the World Health Organization (WHO) in 1987 to raise awareness of the harms caused by tobacco products to people, public health, communities and the environment. World No Tobacco Day aims to draw attention to the business practices of tobacco companies to capture generations of consumers and their interference in policy decisions that undermines effective tobacco control. 1. https://www.who.int/news-room/fact-sheets/detail/tobaccoAn unusual case of old myocardial infarction (Heart attack) with multivessel coronary artery disease (many blockages in arteries supplying the heart) was successfully treated in the cardiology department at SGPGIMS, Lucknow. A 52-year-oldmale patient, belonging to the lower socio-economical class, was admitted in the MICU with history of old extensive anterior wall myocardial infarction in 2014. Outside coronary angiography was done which revealed 100% blocked left anterior descending (main) artery. But due to financial constraints, patient was not able to get the angioplasty (ballooning) of the artery and was getting medical treatment with off and on chest pain (angina). In February 2022, he had sudden onset prolonged resting chest pain with profuse sweating, diagnosedas Inferior wall Myocardial infarction (second heart attack), admitted in a critical condition to cardiology medical intensive care unit(MICU) at SGPGIMS under the care of Professor Dr Satyendra Tewari. After routine investigation, coronary angiography was performed, and itrevealed chronic totally occluded left anterior descending (main)arterywhich was blocked for the last 8 years and critical stenosis of the right coronary artery with poor pumping capacity of the heart. Since the patient was very sick, Professor Tewari decided to openright coronary artery firstwhich was opened up with the balloon and stented successfully.Since the left side of the heart shows viability, so patient was taken up to open the chronic totally occluded (CTO) left anterior descending (main)artery which was 100% occluded for the last 8 years. The difficulty was to locate the site of the blockade. With the use of advanced imaging that is,IntraVascular UltraSound technique (IVUS), the site of chronically occluded blockade was located with the use of ultrasound guided catheter which has a high-resolution camera at the tip which helps in accurately locating the site of the blockade. By the help of extra stiff wires which was passed through the blocked artery and gradual ballooning and successful stenting of the chronic total occluded artery was done. The intervention team comprise of Dr Roopali Khanna, additional professor in the cardiology department. Dr Roopali Khanna helped in intravascular ultrasound imaging (IVUS) which requires lot of experience and precision in locating the site of the chronic totally blocked artery which was present for the last 8 years. Patient was discharged in a stable condition after two days. The head of the department Professor Dr Aditya Kapoor advocates the use of high technology procedures specially Rota ablation,IVUS imaging, Optical coherence tomography andFFRin most of the complex coronary interventions. Professor Dr Aditya Kapoor said that all these high technology equipments are very well available in the department which are not only expensive but also present in a very few centers in the country. Dr Satyendra Tewari informed that these complex coronary interventions are being done routinely in the cardiology department. Dr Satyendra Tewari emphasized that even patients from low socio-economic status can avail of these technological advances through government-aided financial schemes. As patient was financially poor, our respected directorProfessor Dr R K Dhiman had helped the patient in arranging the finances in a very short time. The director Professor Dr R K Dhimancongratulated the team for the success of the high technology supported intervention.

Worldwide, over 1.4 million patients suffer from healthcare associated infections (HAIs) at any time. HAIs have tremendous implications in terms of associated mortality, morbidity, adverse patient outcomes, increased cost of treatment, and social impact1. The WHO guidelines on hand hygiene in healthcare (2009) suggests that hand hygiene is the single most important measure for prevention of Healthcare Associated Infection. When a health facility's "quality and safety climate or culture" values hand hygiene and infection prevention and control (IPC), it results in both patients and health workers feeling protected and cared for. As a part of a major global effort to improve hand hygiene in health care, led by WHO to support health-care workers, World Hand Hygiene Day is celebrated on 5th May every year. The theme for World Hand Hygiene Day, 5th May 2022, is focused on recognizing that the betterment of a facility's climate or culture of safety and quality can be brought about by cleaning our hands. The Department of Cardiology at Sanjay Gandhi PGIMS added another feather in its cap of achievements by performing the first case of Rotablation using the ROTAPRO system by the radial route last week.
Rotablation is a highly specialized technique of treating cases of coronary artery disease which have extensive calcium deposition in the arteries of the heart. It involves insertion of a small rotating drill like catheter inside the heart vessels under fluoroscopic guidance. The catheter then cuts the calcium, thus facilitating easy insertion and adequate expansion of coronary stents.
The procedure was performed on a middle aged female with diabetes and hypertension who had chest pain for last 6 months. Her coronary angiography revealed dense calcium deposits in the major artery of the heart (the left anterior descending artery ie the LAD).
Prof Aditya Kapoor, Head of Department of Cardiology, elaborating the procedure said that in such cases, the plaque in the artery become extremely rigid and hard and a simple angioplasty balloon cannot completely open the artery. Hence it was decided to use rotablation to optimally manage this case using the rotational burr that generally rotates up to speeds of 250,000 revolutions per minute (rpm, typically 140,000-165,000 rpm are used).
Prof Satyendra Tewari said that the ROTAPRO system which has all in-built controls now directly in the operator?s hands on a digital console (as opposed to older systems which had foot operated control panels), makes the procedure more user friendly.
Dr Roopali Khanna, Addl Prof, mentioned that since calcified coronary arteries represent a technical challenge for conventional angioplasty, advent of ROTAPRO means that this technology can be extended to a wider spectrum of patients, hence offering them the benefit of this technology.
Dr Ankit Sahu, Asst Prof, who was part of the interventional team that performed the procedure, also opined that doing such procedures by the hand (radial route) helped in earlier mobilization of patients and is likely to reduce vascular complications.
In this case, the patient was discharged 1 day after the successful procedure. Director, SGPGI, Professor R K Dhiman congratulated the team for the outstanding work.
The Department of Neurology, SGPGI organised an awareness programme on "World Parkinson's Day" on 11th April at Lecture Theatre 1 to create awareness about the disease.
World Parkinson Day is celebrated all over the world to mark the birth anniversary of Dr James Parkinson who gave the first description of this disease. Parkinson's disease is a disorder of the central nervous system which is caused by the loss of nerve cellsin the brain which causes dopamine level to drop leading to the symptoms of Parkinson's. Parkinson's disease often starts with a tremor in hands and legs and gradually progresses to slow body movement and stiffness in limbs and loss of balance. About 0.1% population of India suffer from Parkinson disease.
Genetic and environmental factors, pollution, exposure to insecticides may be responsible for this condition. In majority of cases, when its reasons are not known, it is called Idiopathic Parkinson disease.
To disseminate information about the disease, the awareness program was organized in hybrid mode. The inaugural session started with invocation to Goddess Saraswati followed by welcome address by Professor Sunil Pradhan, Head, department of Neurology. Dr Pradhan threw light on the medical literature available in our ancient veda on Parkinson's disease. He said that Charak Samhita and Atreyan Samhita have mention of the symptoms, reasons and treatment of Parkinson's disease which was described by Dr. Parkinson years later in 1817.CMS, SGPGI, Professor Gaurav Agarwal emphasizedthe need to have more such awareness programmes in the future. Guest of Honour, Dr. Navneet Kumar, former Principal, Kannauj Medical College and GSVM, Kanpur addressed the gathering. Speaking on the occasion, he said, that Parkinson's disease is an ancient disease and some ancient herbs to treat the disease have some contents which are similar to modern medicines.
Speaking on the occasion, Director SGPGI, and Chief Guest of the programme Professor R K Dhiman congratulated the department for organizing the awareness program and emphasized the need to have such programs more often.
This was followed by price distribution to the winners of Parkinson day contest in which patients with the disease participated in various activities and were given 1st, 2nd, 3rd and consolation prizes. All the guests participated in signing on the Parkinson's wall made with posters with motivational messages for people suffering from Parkinson's disease.
The second half of the programme was devoted to lectures on every aspect of Parkinson's disease. Elaborating the extent of problem in India, Prof. Sunil Pradhan said that around ten lakh people from India are suffering from Parkinson's.
Causes of Parkinson's disease were explained by Dr Navneet Kumar. Family history, environmental factors, no physical activity may be reasons for this disease.
Dr. Sanjeev Jha, Professor, Department of Neurology, shared his personal experiences dealing with patients with Parkinson's. "Positivity and endurance arethe keyword to overcome any issue or ailment," he said. A doctor should have empathy towards such patients and should share their grief.
Prof & Head, Department of Neurosurgery, Professor Rajkumar spoke on role of Deep Brain Stimulation in Parkinson's disease. He said that when the side effects of the medicines appear and when the effectiveness of the medicines decrease, then comes the role of surgery.
Dr. Ruchika Tandon, Department of Neurology along with the resident doctors demonstrated exercises which would be helpful in decreasing stress and reducing the symptoms of Parkinson’s disease. She also had a music video for motivating people suffering from the disease.
Professor V K Paliwal proposed the vote of thanks. The programme ended with high tea.
Cardiologist in SGPGI has performed pacemaker implantation in a severely obese patient weighing 150Kg . The procedure was performed in a 61-year-old male who had complete heart block. Patient is a resident of Lucknow and presented to SGPGI emergency 5 days back with complaint of breathlessness. On evaluation he was found to have low pulse rate and was advised pacemaker implantation. Looking at his weight, patient was high risk for the procedure. Dr Roopali Khanna, Additional Professor in the Department of Cardiology in SGPGI, told that there are several challenges in performing the procedure in obese patients. Due to obesity and fat tissue, it becomes difficult to puncture the vein in the neck through which pacemaker leads need to be inserted. Normally 5cm length needle is used to puncture but due to his weight and big body built, a special larger needle was used to puncture the vein.Occasionally, localization of vein by ultrasound can also be helpful. These techniques can help to tackle the problems faced for these type of patients, Dr Satyendra Tewari informed. Dr Aditya Kapoor, Head, Department of Cardiology told that very few cases of pacemaker implantation in morbid obese are reported worldwide. After successful procedure, the patient is also able to walk and has been discharged from the hospital within 2 days.

Births and Deaths are the two most important vital events that define life of an individual and the right to be counted at both ends of the life spectrum is equally essential for social inclusion. Not only do they describe the legal existence of an individual, registration of these events is a source of basic vital data of the population to which they belong.
Civil Registration is a driver for accessing fundamental rights. It was in 1969 that a Central Act known as the Registration of Births and Deaths (RBD) Act, 1969 was enacted to overcome the then prevalent diverse legal provisions for registration of births and deaths in the country. The Act unified the system of registration throughout the country and made registration of births and deaths compulsory. It also defined the procedures, machinery, principles, personnel and penalties connected with the enforcement of the Act.
To ensure that the registration of Births and Deaths is carried out in the country as per the RBD Act, it should be necessary that persons engaged with the registration work are fully conversant with the provisions in the Act and the Rules made there under from time to time. It is the responsibility of the officer in charge or any person authorised by her/him to report the births and deaths that occur in hospitals, nursing homes, health centres etc. to the Registrar of Births and Deaths of the concerned area. Health care workers who have attended or were present at the time of birth or death are duty bound to notify the concerned Registrar about the occurrence of the event.

Communication plays a very important role in our day to day life in putting the information across. Effective communication ensures mutual understanding among us which minimizes chance of misunderstanding. In a professional surrounding, the better we communicate, the more credibility we have with our colleagues, juniors and seniors. Clarity & brevity of communication and active listening are the traits which can be learnt through practice.
The need to inculcate good communication skills in our staff was strongly felt to minimize friction with patients and their attendants which occur due to poor communication and lack of coordination.
A workshop on "Communication Skills" was organized by the department of Public Relations on 14th February at Dr. Hargobind Khurana Auditorium, Lecture Theatre which was attended by Receptionists, Junior Reception Officers, Public Relations Officers, Medical Social Workers, Nursing Staff and Data Entry Operators of Reception and Registration Desk.
Speaking on the occasion, Professor R K Dhiman, Director, SGPGI, underlined the importance of communication skills in any hospital in order to understand patient's perspective and their emotions and in this way extending all possible help to them. Losing temper can cause physical and mental stress to the staff also as nobody wants unpleasantness. As we all have to work for longer time in hospital, it is all the more important to stay physically, mentally and emotionally fit. He added that showing compassion is vital in communication as compassion can help in making the environment congenial.
Medical Superintendant, Professor V K Paliwal expressed his views on "Listening Skills". He very effectively explained the need to listen actively. Differentiating between hearing and listening he said that ability to receive information, interpret and generate the appropriate response is listening. Communication skills include active listening where one has to be present, patient and positive while listening to the patient. "Empathizing with the patient is also an important part of active listening" he said.
"How important is the effective communication in hospital" was elaborated by Mrs. Kusum Yadav, Public Relations Officer. She explained the 7 Cs of effective communication i.e. Clear, Concise, Concrete, Correct, Coherent, Complete and Courteous.
Explaining further she said, "Effective Communication Skills in Healthcare prepare us to know the social determinants of the patients and their attendants and thus creating synergy by coordinating with different departments. Proper coordination with every related department will minimize their problems." People differ in their needs for communication. Some want to be listened to. Conventional open thyroid surgery leaves a scar in the neck which is considered undesirable by young women, girls & boys. Robotic surgery provides aesthetic and cosmetic surgery & does not leave a scar in the neck; instead it leaves scar near the arm pit which is hidden within the clothes. Trans oral method involves approaching &removing the gland through the mouth; hence there is no scar in the body. It is a scar-less surgery. This operation was performed in SGPGI by Prof Amit Agarwal, Head of Department of Endocrine Surgery & In-charge SGPGI Robotic Surgery program on a 35- year- old patient who did not want a scar in the neck. Cardiologist in SGPGI has performed balloon dilatation of heart valve through neck veins which is a rare procedure. The procedure was performed in a 25-year-old lady who had marked narrowing of left heart valve. This procedure is commonly done through leg veins. Procedure through the leg vein was not possible in this patient due to abnormal leg veins and hence she needed an open-heart surgery.
Dr Roopali Khanna, Additional Professor in the Department of Cardiology in SGPGI, accepted the challenge and admitted the patients. Dr RoopaliKhanna along with her anesthetic colleague Dr AmitRastogi, took a daring decision to perform this procedure through the neck veins which need an exceptional experience and expertise.
